Finding Fortune in Traditional Medicine

Traditional medicines and treatments could help provide the next wave of affordable drugs and medicines for the world. But a phenomenon known as ‘bio-prospecting’ – in which global companies grab a stake in these once-free medicines – has been placing traditional medicines out of reach of Southern entrepreneurs. Pharmaceutical patents (http://en.wikipedia.org/wiki/Patents) taken out by international drug companies are making traditional medicines expensive and inaccessible to the poor.

Indian scientists have identified more than 5000 bio-prospecting patents, worth some US $150 million, taken out by companies outside India.

Now governments in countries like India are moving to protect these recipes and the plants and animals they are made from.

The Indian government has labelled 200,000 traditional treatments as public property and free for anyone to use. These treatments are key parts of the 5000-year-old Indian health system called Ayurvedic medicine (http://en.wikipedia.org/wiki/Ayurveda) – ayur means health in Sanskrit, veda means wisdom.

“We began to ask why multinational companies were spending millions of dollars to patent treatments that so many lobbies in Europe deny work at all,” said Dr. Vinod Kumar Gupta, head of the Traditional Knowledge Digital Library, which lists in encyclopaedic detail the 200,000 treatments.

“If you can take a natural remedy and isolate the active ingredient then you just need drug trials and the marketing. Traditional medicine could herald a new age of cheap drugs,” Gupta told The Guardian..

Currently, it is very expensive to follow the Western approach to developing drugs. A so-called “blockbuster drug” can cost US $15 billion and take 15 years to bring to the market. With patents lasting 20 years, a drug company can have as little as five years to recover its development costs. This helps explain the high prices for drugs.

Unlike traditional healers in the South, multinational corporations can marshal the money, time and legal resources to file patents.

In the past, India has fought expensive and lengthy battles to revoke patents on traditional remedies. One example is the battle over the popular Indian spice turmeric powder (used for healing wounds, among other things). A patent awarded to the University of Mississippi in 1995 was successfully withdrawn after a legal battle by the Indian government.

The Indian government’s move to make traditional medicines and therapies public property promises to unleash a new wave of natural remedies and drugs and to expand the market for Southern health entrepreneurs drawing on traditional knowledge and recipes.

As the world’s economy continues to suffer, finding new ways to earn incomes and spark a whole new generation of businesses will be crucial to recovery.

The World Health Organization (WHO) defines traditional medicine as “the sum total of knowledge, skills and practices based on the theories, beliefs and experiences indigenous to different cultures that are used to maintain health, as well as to prevent, diagnose, improve or treat physical and mental illnesses.”

The importance of traditional medicines in primary health care can be seen in Asia and Africa, where its usage reaches 80 percent of the population in some countries (WHO). Herbal medicines alone are worth billions of dollars a year in sales. Examples of traditional remedies include antimalarial drugs developed from the discovery and isolation of artemisinin from Artemisia annua L., a plant used in China for almost 2000 years. In 2003, doctors found scientific evidence supporting the use of traditional Ghanaian plants to help wounds heal. Parts of the African tulip tree and the Secamone afzelli are made into pastes which are applied to wounds.

The downside of traditional medicine is the urgent need for better regulation and safety standards. While more than 100 countries have regulations for herbal medicines, counterfeit, poor quality or adulterated herbal medicines are still a major problem.

Herbal treatments are the most popular form of traditional medicine, and are highly lucrative in the international marketplace. Annual revenues in Western Europe reached US $5 billion in 2003-2004.. In China, sales of products totalled US $14 billion in 2005. Herbal medicine revenue in Brazil was US $160 million in 2007 (WHO).

One initiative is ensuring there is a solid future for traditional medicine in India. Charity Bodytree India, set up in 2004 by a group of health, human rights and education workers, addresses issues surrounding access to health care and the disappearing traditional medical practices amongst isolated indigenous communities. Bodytree has established a successful educational programme that trains young people from different indigenous communities to become community health workers and operates programmes of health education for community groups (http://www.bodytree.org/index.html).

Almost four-fifths of India’s billion people use traditional medicine and there are 430,000 Ayurvedic medical practitioners registered by the government in the country. The department overseeing the traditional medical industry, known as Ayush, has a budget of 10 billion rupees (US $260 million).

In the state of Kerala in India’s South, Ayurveda medical tourism has become a good income generator. And it is so popular in the nearby nation of Sri Lanka, hotels can have Ayurveda included in the name.

Indian entrepreneurs are drawing on increasing awareness of the importance of healthy living and rising interest in vegetarian diets – what were once holidays are now health experiences. With global obesity rates rapidly rising, along with the attended diseases like cancer and diabetes, more and more people are looking for a dramatic change to their eating and lifestyle habits to ensure long-term health. And traditional medicine has solutions.

Southern Drink Challenges Corporate Dominance

Across the global South, its thirsty people have long been a target market for Northern drinks companies. The ubiquity of the American soft drink Coca Cola, or even its rival Pepsi Cola, is testimony to that. Even the most remote village on the impoverished island of Haiti can offer an ice-cold Coke.

But the marketing power of these companies has a down side: it has pushed aside local drink brands based on traditional formulations. But in some countries, local brands are fighting back.

In India, the Cow Protection Department of the Rashtriya Swayamsevak (called RSS) based in Hardwar (www.hardwar.com), one of the four holy cities on the River Ganges, has produced a soft drink made from recycled cow urine. They call it ‘gau jal’ (Sanskrit for ‘cow water’) and it is set for a launch at the end of 2009.

The urine is highly processed to make the drink. “Don’t worry, it won’t smell like urine and will be tasty too,” Om Prakash told the Daily Mail. “Its unique selling point will be that it’s going to be very healthy. It won’t be like carbonated drinks and will be devoid of any toxins.”

The price will be less than American brands such as Coca Cola.

“We’re going to give them good competition as our drink is good for mankind,” he continued. “We may also think of exporting it.”

The drink contains not only cow urine but a blend of medicinal and ayurvedic herbs. Ayurveda is the 5,000-year-old ancient Indian health system.

The RSS was founded in 1925 and claims to have eight million members.

Cows are sacred to India’s Hindu population and killing them is illegal in many parts of India.Finding ways to make a living from cows’ waste products is common. Cow dung (manure) is already used as a fertilizer in villages. It is claimed the new soda pop will help with cancer, obesity and liver disease.

Another drink that has been consumed for its health-giving properties is Mongolian mare’s (female horse) milk. Studies by female scientists from Mongolia, South Korea and China for UNDP in the late 1990s found the milk was packed with vitamins and minerals and effective in treating liver diseases, cancer, intestine inflammations and tuberculosis.

Mongolians have used mare’s milk for centuries in their traditional diet. The drink, called airag in Mongolian, is consumed especially during traditional holidays.

There are eight times as many horses in Mongolia as the human population, which numbers 2.7 million, so the potential for this drink is enormous. The Food and Biotechnology Institute of the Mongolian University of Science and Technology (www.must.edu.mn/beta_new/) in association with the Swiss International Development Agency (www.sdc.admin.ch), has been developing technology to process mare’s milk, and make value-added products with it to create rural jobs. Under the project, eight kinds of beauty products have been manufactured so far using mare’s milk.

Thai Organic Supermarkets Seek to Improve Health

A Thai business is working hard to expand access to organic food in the country. It sees this as part of a wider campaign to improve health in the country – and its success has caught the attention of the government, which wants to turn Thailand into a global health destination.

The Lemon Farm chain run by Suwanna Langnamsank (http://www.lemonfarm.com/lmf/) was started 13 years ago and has grown to nine organic supermarkets in the capital, Bangkok. Lemon Farm works with 200 organic farms in Thailand and employs 160 people.

Organic food (http://en.wikipedia.org/wiki/Organic_food) – grown without chemicals and artificial fertilizers and not irradiated or subjected to other tampering – is believed by many to be healthier because it avoids the harmful effects of accumulating chemicals. It is also thought to be richer in vitamins and minerals because of the use of non-chemical fertilizers on the soil.

Lemon Farm sells made-in-Thailand organic vegetables and fruit, natural gift sets, soap and tea. There are also macrobiotic cafes in the supermarkets called Be Organic.  A macrobiotic diet avoids foods containing toxins (http://www.cancerresearchuk.org/cancer-help/about-cancer/treatment/complementary-alternative/therapies/macrobiotic-diet).

The supermarkets use eye-pleasing modern design to set themselves apart from more conventional supermarkets.

According to Lemon Farm’s website, it is a social enterprise and practices fair trade. It is using market-driven solutions to increase the availability of healthy food in the country. It seeks to support small-scale farmers and champion change in farming methods, encouraging a move away from dependence on harmful chemicals that damage human health and the environment and promoting “agricultural and economic self-sufficiency”.

The macrobiotic restaurant operates to six values, among them using fresh vegetables and only using produce from associated farms. The restaurants do not use added sugar, they cook using a pressure cooker, and use natural ingredients such as sea salt, ginger, fermented soy sauce and natural miso. They do not use any artificial preservatives or flavour enhancers such as monosodium glutamate (MSG), a common practice in Asian cooking.

Lemon Farm’s success as an organic food pioneer has caught the attention of the Thai government. The Ministry of Commerce (http://www2.moc.go.th/main.php?filename=index_design4_en) has contracted Lemon Farm to join its campaign to offer organic food in schools and hospitals.

By promoting organic food, the government is hoping to boost farmers’ incomes while improving health in the country and bolstering the country’s thriving medical services industry serving foreign patients.

“We need to promote healthy food and a healthy environment,” Piramol Charoenpao, deputy permanent secretary at the Ministry of Commerce, told Monocle magazine. “Thailand is a medical hub. The idea is to have retreat-style hospitals serving organic food. We’re increasing organic food production and educating people about it.”

Thailand has already built a good reputation with its medical and health services. More than 1.6 million non-Thais are treated in Thai hospitals annually, with an estimated 500,000 travelling specifically for medical treatment (The Guardian).

Former Thai Prime Minister Thaksin Shinawatra mooted the idea of making the country an international leader in medical tourism in 2003. It is expected providing medical services to overseas patients will make the country US $3.3 billion by 2015 (The Guardian).
 
It is hoped that offering organic food in hospitals and health facilities will boost the attractiveness and effectiveness of using health services in Thailand.

Medical tourism is considered one of the fastest-growing sectors in the world. Estimates place it as a market worth US $100 billion. Three countries that compete in this market by offering medical services in the English language include India, Singapore and Thailand. They compete by offering services comparable to wealthier countries but at considerably less cost.

Lemon Farm says it is on a mission to develop the marketplace for organic food in Thailand by educating consumers and producing “innovative natural food”.  It looks like it has already made a big impact.

Indian Toilet Pioneer Champions Good Ideas

Access to adequate sanitation and toilet facilities is critical to making development gains. Yet this simple fact of life often gets overlooked, especially in fast-growing cities where populations are on the rise or in transit. Out of an estimated 2.6 billion people in the world without toilets, two-thirds are in southern and eastern Asia (World Toilet Organization).

It is easy to take toilet technology for granted in developed countries, but in the fast-growing urban world of the global South, increasing access will be the dividing line between a future of good human health and dignity, or misery and poor health. The biggest gains in human health always come about once people have access to clean water and sanitation. Yet this proven fact gets lost in many places for a wide variety of reasons.

One country currently failing to meet the needs of its population is India. According to the McKinsey Global Institute, by 2030, 70 percent of India’s jobs will be created in its cities, and 590 million Indians will be city-dwellers. An enormous infrastructure task lies ahead for India: a city the size of Chicago needs to be built every year. But so far this challenge is not being met, leaving the country with the largest number of urban slum dwellers anywhere in the world. Housing is just not keeping up with populations’ needs.

As K.T. Ravindran, a professor of urban development, told the New York Times: “We require radical rethinking about urban development. It is not that there are no ideas. It is that there is no implementation of those ideas.”

It is this ability to act that makes the Sulabh International Social Service Organization stand out. The Indian non-governmental organization (NGO) sees itself as a movement and is a passionate advocate for toilets and toilet innovation for the poor and underserved.

Sulabh was founded in 1970 by Dr. Bindeshwar Pathak, who saw the vast task ahead. “I thought the challenges to provide toilet facilities have been overcome in rich countries; it has still to be met in developing countries like India,” he said.

So far, Sulabh has brought together 50,000 volunteers across the country to build toilets and sanitation facilities.

The organization’s success flows from understanding that it needs to do more than supply the ‘hardware’ of the toilets; it also needs to address the ‘software’: ideas and innovation and concepts.

The organization has directly built 1.2 million household toilets – but the government of India has built a further 54 million toilets based on the designs made by Sulabh. It’s an example of a good idea multiplying its impact when picked up by others.

While 10 million Indians use a Sulabh-built sanitation facility each day, according to the group’s website, an estimated 300 million are using a toilet based on Sulabh’s designs.

Most influential is Sulabh’s two-pit, pour-flush toilet (www.sulabhenvis.nic.in/Sulabhtechnology.htm). It consists of a toilet pan with a steep slope using gravity to flush the pan. Water is poured in to the pan to flush the toilet and the waste goes into either one of two pits. As one pit fills up with waste, waste is diverted to the second pit. After around 18 months, the first, filled pit’s waste becomes a safe, organic fertiliser suitable for agriculture and the fertiliser’s value covers the cost of emptying the pit. The successful design has been evaluated and approved by UNDP and the World Bank.

Sulabh has also been designing ways to get power and energy from toilets, building 200 biogas plants that turn the gas generated from the human excrement deposited in the toilets into a source of energy. Biogas (http://en.wikipedia.org/wiki/Biogas) is a clean-burning gas that can be made from animal, plant and human waste with the right technology and is a green solution to the need for gas to cook and run electricity generators.

Pride of place for the NGO is its vast toilet and bath complex at the holy shrine of Shri Sai Baba in Shirdi, Maharashtra (http://en.wikipedia.org/wiki/Maharashtra). Millions flock to the shrine every year, but it lacked proper sanitation facilities. To solve this problem Sulabh’s local branch has built a vast complex occupying two acres. The brightly coloured and palace-like facility has 120 toilets, 108 bathing cubicles, six dressing rooms, and urinals and can serve 30,000 people a day. There are telephones and 5,000 lockers for tourists to keep possessions safe.

There are also three biogas plants connected to the facility, generating electricity and hot water for bathing used by the toilet and bath complex. This solves the puzzle of how to fund the utilities. Water discharged from the facility is used to irrigate the surrounding green spaces.

Sulabh has also built a museum dedicated to toilets and toilet technology (http://www.sulabhtoiletmuseum.org). The museum places the toilet as a critical part of human civilisation and shows how it fits in with the cultural context of India. Toilets and toilet designs from around the world and throughout history are gathered together and make a fascinating journey through this essential human need.
